Payroll Forms

All incoming employees must complete the payroll forms below. Questions about form completion can be directed to the English Business Office at 301-405-3200.

ALL FORMS MUST BE COMPLETED IN BLACK INK. THE FORMS CANNOT CONTAIN ANY "CROSS OUTS" OR "SCRIBBLES." IF AN ERROR IS MADE, PLEASE START OVER WITH A NEW FORM.

I-9 Form
  1. I-9 Employment Eligilbility Verification form
      You must include appropriate forms of identification listed on the last page

W-4 Forms
  Instructions
  2. MD Resident W-4 form (for those residing elsewhere, including PA and VA)
  2. DC Resident W-4 form
  2. WV Resident W-4 form

Central Payroll Forms
  3. Direct Deposit form
  4. Employee Information/Data Collection Form
      Be sure to include dates (month/year) for degrees and previous employment

Health Forms
Complete information on Health Benefits can be found on the State of Maryland's Employee site. Health benefits are optional.  Any regular employees that are 50% or more FTE (with the exception of TAs who can be 25%) are entitled to health benefits.  The required forms are available on the Health Benefits and Retirement Forms Page.

Retirement Forms
Incoming faculty, staff, and lecturers that are 50% FTE or greater must choose a retirement plan.
Enrollment within the first week of your appointment is required. Additional information on the options is available on the UHR Benefits WebsiteThe required forms are available on the Health Benefits and Retirement Forms Page.

Change of Address
If you move while you're employed with the University of Maryland, you must complete the following two forms:
   1.  Payroll change of address form
   2.  Updated copy of W4 for your new state

The instruction sheet provides information on how to submit requests online via ELF for classes at College Park. If you are taking classes at a USM campus other than College Park, please submit the appropriate form to the English Business Office in 2119 Tawes Hall. Requests are made prior to the semester you are taking classes; they must be completed each semester. Additional information regarding tuition remission can be found on the UHR Benefits Website.

University ID Card
An I.D. card can be obtained by visiting the Mitchell Building on or after your appointment start date. You do not need any forms or letters from the department. Bring another form of identification and they will be able to locate you in the system.

University Accounts
You can setup your new University of Maryland e-mail account through the Accounts Activation Page.
